Step 01

  • Staff / Visitors requested fill in Intake Forms (New Registration, etc), after visit schedule is confirmed.

Step 02

  • Staff, Customers, Patients or Visitors fill in the forms and share the digitally signed form back to the Administrator.

Step 03

  • The office Admin receives the duly self-attested (digital eSign) forms which automatically stored/saved in the Organization’s CMS/ERMS/HRMS.
